will find on the Insolvency Service website. See pp 128-31 for a guide to completing it. Fill it in carefully, and note the following: – the petition itself does not require dating,

signing or witnessing;

– you can only claim the debt as stated in your statutory demand (no ‘Oh, and by the way …’);

– if you have a county court judgment, you need to confirm that the debtor has not paid any money to the court.
