ABSTRACT
A female patient (SC) killed her social worker and the subsequent inquiry (Department of Health and Social Security, 1988) led to the Royal College of Psychiatrists’ (1991) guidelines on aftercare. A prominent feature of this case was rejection of a patient by mental health services on the mistaken grounds that the diagnosis was personality disorder rather than schizophrenia. There was a failure to respond adequately to earlier acts of violence.
