ABSTRACT
If the matter is urgent, you should organise for the application to be issued in person at your local court as soon as possible, preferably on the same day. You must provide the following: (a) the original and at least two copies of completed
form FL401; (b) the original and at least two copies of your
client’s sworn statement; (c) form FL402 (notice of proceedings) plus two
copies; (d) the fee or your legal aid certificate. Where the
latter is yet not available, the person issuing must give an undertaking to file it with the court as soon as it is available.
