I have reprinted the responses of all of the patients available by mail who were described in the case reports of this book, and who wanted to reply with their own points of view. My letter to each of them ran as follows:

I would also like you to write a few paragraphs on your own point of view of this work if it differs from or augments the text as written. I will include your addition in a chapter titled “Follow-Up Judgments by Patients Described in the Text.”
