ABSTRACT
The dominoes in Figure 7.1 represent the sequence of causes (events or situations) leading to an accident that results in injury or damage. If a domino falls it will knock over the rest until eventually the final domino
falls, i.e. an accident happens. If any of those dominoes (causes) is missing, i.e. the correct safety action has been taken, there will be no accident.
