ABSTRACT
The American Society of Health-System Pharmacists (ASHP) defines pharmaceutical care as the direct, responsible provision of medicationrelated care for the purpose of achieving definite outcomes that improve a patient’s quality of life. The Joint Commission on Accreditation of Healthcare Organizations (JCAHO), which has profoundly affected health and pharmaceutical care, has redefined the accreditation process by placing patient outcomes at the center of their standards.
