ABSTRACT

In this chapter, the author provides detailed instructions on how to implement a prize Contingency Management (CM) program that reinforces abstinence. She describes a 12-week CM protocol that reinforces abstinence from methamphetamine, with twice-weekly urine testing. If the clinician are reinforcing abstinence from another substance, instituting a different duration of reinforcement (e.g., 16 weeks), or implementing a system that monitors samples more frequently (e.g., Monday-Wednesday-Friday), they will need to make minor modifications to the procedures described. Therapists can be supportive and offer advice in the context of CM, and do so in a focused manner, directing patients to other clinical resources as needed. The CM therapist’s role does not get intertwined with other clinical issues. On the other hand, if the CM were integrated in the context of a lengthier cognitive-behavioral session, the preceding exchange may have been very similar, but the therapist may have gone on to conduct a functional analysis of drug use.
