ABSTRACT
The debate over the most effective and prudent strategies to reform the U.S. healthcare system rages on. There is virtual consensus over the desired goals: expand insurance coverage to the millions currently without coverage, assure access to appropriate and timely care, achieve higher quality of care with improved outcomes, and reduce overall costs. The problems are easy to see; the solutions are not so readily apparent.
