ABSTRACT

The progress note is to the physician what a canvas is to the painter. It is a medium for the physician to document his particular encounter with a patient. It is a permanent document that is used by the treating physician and other physicians as a basis of reference, by plaintiff attorneys in litigation cases, and by hospital review committees for quality and assurance. The physician must be neat, clearly descriptive, and non-offensive in his presentation. The physician must also show logical deductive and reasoning abilities in his decisions.
