Life expectancy has probably increased by one-half since 1940 in most low-income countries.1 This extraordinary extension of life occurred in high-income countries more gradually at a later stage in their economic development. Such an important modification in man’s environment creates widespread opportunities for improved welfare but at the same time necessitates modifications in individual behavior and social institu­ tions to exploit these new opportunities fully.
