ABSTRACT

Subsequent to the initiation of the site emergency or field response plan, and the implementation of any immediate corrective actions necessary to prevent escalation of the incident, a critical step in the pre-work needed before an incident investigation process is formally commenced is scene preservation and early data-gathering. A formal data- and information-gathering step is usually reserved for the more detailed incident investigations. A People, Environment, Equipment, Procedures/documentation and Organization (PEEPO) is used to help an investigation team think about what information, data and records they wish to collect – to help them understand what happened in an incident. Carrying out a PEEPO is one of the simplest tasks that need to be carried out by the incident investigation team. This chapter describes the investigation process for determining Work-As-Done, Work-As-Normal and Work-As-Intended for Outcome Analyses and incident investigations.
