Few epidemiological data of overuse injuries in adolescent as well as professional cyclists demonstrate sports-related symptoms at the lower back as well as the lower extremity. This chapter presents some case reports of adolescent elite cyclist. Regular application of specific prevention programs results in a distinct reduction of overuse injury rates. In a group of professional road cyclists, low back pain was found to be the most prevalent overuse injury with 58 percent. Lower extremity pathologies among young elite cyclists frequently originate from tendinous structures around the knee. Moreover, low back pain in adolescence can be referred to a variety of causes, arising from symptoms of bony, muscular, tendinous, joint, ligament or nerve structures. Treatment strategies should naturally be focused specifically due to the diagnoses made. The treatment algorithm for patellar tendinopathy should be based on a multifactorial approach. Boxes 8.1 to 8.3 aim to focus on evidence-based exercises for the prevention of common overuse injuries in adolescent cyclists.
