ABSTRACT
This incident happened at a Department of Energy research lab. Some of the take aways from this incident are of greater importance than the accident itself. They are:
Failure of on-the-job training/mentoring
Failure to make people aware of policies
People will always do things you could never imagine them doing; in other words, there is no limit to stupid actions
