ABSTRACT

A method to monitor and provide feedback on process safety system activities and performance is essential for an effective process safety program. This chapter discusses the primary methods of feedback for process safety is periodic audits of process safety system implementation and performance and the development of appropriate key performance indicators (KPIs) or metrics. Sensitivity to operations and effective organizational learning are both related and overlapping parts of the feedback systems involving purposeful monitoring and improvement of process safety program performance that must occur if serious process incidents are to be prevented. Metrics must be collected periodically, reviewed by management and other affected personnel, and based on absolute values or trends of the metrics data, either supporting or corrective actions must be taken to maintain positive results or improve negative results. Audit findings involve gaps observed in the design, implementation, or performance of process safety programs relative to both regulatory and corporate/business/facility requirements.
