ABSTRACT

This introduction presents an overview of key concepts discussed in subsequent chapters of this book. The book analyses accidents as a sequence of events evolving from lack of control to loss of control, primarily in terms of energy in an industrial system, and finally to where loss occurs through personal injury or damage to the environment or to material assets. It explains health, safety, and environment (HSE) are used in relation to management, which goes beyond the prevention of unwanted occurrences due to loss of control. The book also presents case studies dealing with the application of various methods and tools in the prevention of accidents in three industries - oil and gas, hydropower, and transportation. The book presents many channels for accident prevention through experience feedback, such as reporting unwanted occurrences, workplace inspections, safety audits, and accident investigations. Rockwell pioneered the implementation of statistical quality control techniques in the field of safety management.
