ABSTRACT

This chapter discusses safety performance indicators based on causal factors, according to the basic accident analysis framework. It distinguishes between performance indicators based on the measurement of contributing factors at the workplace and general and health, safety, and environment (HSE) management factors. The chapter also tabulates a sample of indicators related to the workplace level. It classifies them according to the man, technology, and organisation (MTO) model, which is similar to the classification of performance indicators of 'workplace precaution factors' according to the fishbone diagram. The use of the international safety rating system (ISRS) auditing tool is one example where a crescendo of activities to improve HSE management system elements just prior to an audit were reported. Typically, top management sets goals for improvements in the ISRS scores for the following years. In order to meet the goals, middle management applies the ISRS manual as a comprehensive and detailed recipe for development of their internal HSE management system.
