ABSTRACT

Accident models play a vital role in the design of methods for accident investigation and of feedback systems for safety management in general. This chapter categorizes six types of accident models. Five of which are causal-sequence models, process models, energy models, logic tree models, system models, and cognitive models. Many accident models are combinations of these six types of accident models. The chapter deals with a short review of safety culture and focuses on models of safety culture that explain an organisation's informal processes and beliefs and how they affect the risk of accidents. The Management Oversight and Risk Tree (MORT) program was first developed in the 1960s by the US Atomic Energy Commission. In the years since, it has had a great influence on developments in safety management. The thought at that time was to formulate an ideal safety management system from a synthesis of the 41 best accident models and quality assurance techniques then available.
