ABSTRACT

The public health nursing program chosen for study was one of the services provided through the Nursing Bureau of the Syracuse Public Health Department. The Home Care Nursing Program studied was intended not only to give bedside care to the chronically ill but also to coordinate the efforts of the family to care for the patient with a variety of available community resources. The concept of home care for the chronically ill usually includes a complete range of medical and social services as well as continuity of care. The nursing office in which the Home Care Program was housed was open five days a week, and applications for service could be made directly by patients and their families. According to the problem categories used in the core agencies, 14 of the Home Care Program patients were suffering from disorders contingent upon being aged, and 50 were chronically ill.
