ABSTRACT

An accident can be defined in many ways, but they all mean the same thing. Our world is made up of so many hazardous conditions that the only way we can be safe is to recognize and know the hazards inherent in each job or situation encountered. The purpose of an accident investigation is to find the causes of the accident in order that appropriate preventive measures can be taken. Accidents are rarely caused by a single factor, but instead, by several conditions or events that come together at the same time. In some organizations, close-shaves, near-misses and incidents are investigated with the same spirit of inquiry as that of major accidents. The purpose of investigating accidents is to uncover all the details regarding how and why the accident happened. Without accident investigation, we are bound to have the accident be repeated many times without knowing the cause or how the same accident can be prevented.
