ABSTRACT

This chapter describes some incidents which occurred as the result of errors by operating staff. Accidents which occurred as the result of a failure to operate a permit-to-work system correctly are discussed. When liquefied gases are transported, pressure tankers are used. Overfilling does not result in a spillage as the vapour outlet on the top of the tanker is normally connected to a stack or a vapour return line, but it can result in the tanker being completely full of liquid. Hoses, like bellows, are items of equipment that are easily damaged or misused, as the following incidents show, and while better training, instructions, inspections, etc., may reduce the number of incidents. The incidents described below were really the result of sloppy methods of working that is, of poor management. People should not be expected to rely on word of mouth when mishearing or misunderstanding can have serious results. There should be better methods of communication.
