ABSTRACT

The Transportation Safety Board of Canada (TSB) has, on several occasions, expressed its concern over the frequency and the consequences of controlled flight into terrain (CFIT) and loss of control (LOC) accidents. Transport Canada's Director General Civil Aviation convened a study team, not only to evaluate the regulations, but to examine, in broader terms, the safety of flight in reduced visibility and develop measures to prevent CFIT and LOC accidents. Virtually all CFIT accidents are attributable to human error. They are particularly frustrating to investigators and regulators because the errors leading to CFIT have been identified again and again, but the same accidents keep happening. Some reports did not contain enough information to support a complete analysis of error type and identification of antecedents, but in all cases, the group was able to identify one or more unsafe acts and categorise the accidents.
