ABSTRACT

This chapter explains the key principles in the management of complex pediatric colorectal diagnoses. It provides case-based presentations, radiographic images, operative images with multiple choice questions to test knowledge. The chapter presents a case study of a 13-year-old girl, with a long-standing history of constipation. Previous medical management has included high-dose laxatives without success. Cecostomy was placed and antegrade flushes were introduced without success (saline, bisacodyl, and glycerin). Laparoscopic resection of the distal colon (and the proximal rectum in a patient with functional/idiopathic constipation) with continued use of the cecostomy. After colonic resection colon is easier to clean with antegrade flush regimen. In future (in 6-12 months), laxatives could be tried again and are expected to work at a much lower dose than has been tried previously.
