ABSTRACT

The United States spends almost double what other wealthy nations spend on healthcare, with little to no improvement in outcomes (Woolf 2013). Largely due to this fact, and an aging population, the healthcare industry is undergoing rapid and transformational change, including emphasis on fi nding solutions for persistent, costly population health challenges such as obesity and diabetes. Related, as the United States transitions to a value-based healthcare system, health-focused organizations (including public health) and communities are increasingly working collaboratively to improve population health with the desire of achieving lower healthcare costs as well as better health outcomes.
