A 55-year-old woman presents with clear otorrhoea from her right ear. She has had multiple mastoid surgeries in the past for cholesteatoma, the last one about 20 years ago. She has intermittent clear otorrhea from her right ear several times a year since then. On examination, she has a large right mastoid cavity, with a skin-lined mass in the posterior superior aspect, easily seen through the wide meatoplasty (see picture).
