ABSTRACT
I want to discuss the way that bureaucracies allow accidents. While the Reason model is a good general guide to the nature of organisations' vulnerability to an accident, we must go further. I suggest that we think of accidents as falling into general types. In the process of classifying organisations into general types, we will become more aware of the differences in the accidents themselves and also in the ways that we can prevent them.
