ABSTRACT

A method always embodies a set of underlying principles that encapsulate the salient features of the phenomena that it can address. The purpose of the AcciMap method is to identify the factors that caused an accident. The Tripod approach to accident analysis targets the underlying problems that lead to incidents. A Tripod accident analysis focuses initially on the accident mechanism and uses it as a structure to identify the hazard management measures that should have been in place. Systems-Theoretic Accident Model and Processes is an accident analysis method that explains accidents as a result of inadequate control or inadequate enforcement of safety-related constraints rather than as a result of component failures and malfunctions. The Functional Resonance Analysis Method proposes that everyday events and activities can be described in terms of the functions involved without predefining any specific functions or assuming they are organised in a specific way.
