ABSTRACT

This chapter reviews an accident to a DC-10 aircraft which occurred in November 1979, at Mount Erebus in the Antarctic. Before addressing the accident itself, a number of topics are briefly considered, with a view to helping the reader evaluate how best to bridge the gap between abstract and applied approaches to systemic safety. The Erebus accident is subsequently used to illustrate the analytic benefits of the principle and ideas outlined in Chapter 1. The chapter concludes with a brief re-examination of conceptual issues which arise from the earlier discussion.
