ABSTRACT
As Cicero once said "to err is human" but he also went on to observe that "only a fool perseveres in error". In this chapter we will explore the problems of human error, how theories of error have evolved, how to prevent errors occurring and finally how the concepts of systemic failure are the only viable way to investigate incidents in modem technological systems.
