ABSTRACT
In Chapter 2, the basic characteristics of mistakes and how they differed from actions-not-as-planned were outlined. In Chapter 3, I spelled out the arguments leading to the distinction between rule-based and knowledge-based mistakes. This chapter focuses on the ways in which planning can go wrong-at both the individual and collective levels of its formulation.
