ABSTRACT

There are a relatively small number of pathologies that cloud the vitreous so much as to make any meaningful interpretation of the state of the retina impossible. There is worsening pain and very poor vision with examination revealing a panuveitis. Low-grade postoperative endophthalmitis, while not causing a fundus obscuring panuveitis is a post-operative infection which causes grumbling low-grade anterior uveitis. This is usually lumped into the 'post-operative uveitis' box in eye casualty and treated with topical steroids, which transiently improve things, with the situation deteriorating again on stopping the steroids. The principles of investigation and treatment of unknown ocular fungi are the same where knowledge of the systemic condition, any growths from blood cultures and the ocular appearance guide treatment. Initial treatment is with intensive steroid therapy, both topical and systemic, in order to settle the inflammation, followed by vitrectomy to clear retained lens matter.
