ABSTRACT

This chapter presents a case study of a 8-week-old male infant who is presented with worsening vomiting after feeds. The patient has a clinical history suggestive of gastro-oesophageal reflux disease (GORD). Reflux describes the passage of gastric contents into the oesophagus with or without regurgitation and vomiting. Clinical features in infants include feed refusal, recurrent vomiting, poor weight gain, irritability and respiratory problems such as recurrent wheeze or cough. Differentials for recurrent vomiting in this age group include cow's milk protein allergy, intestinal motility disorders, pyloric stenosis, infectious causes such as gastroenteritis and urinary tract infections and metabolic disorders. The parents should be advised to position the child in the upright position for 30 minutes after feeds. In breastfeeding infants with feeding difficulties, ensure that the mother has had a breastfeeding assessment by a trained member of the staff.
