ABSTRACT

This chapter explains the purpose of documentation and how it can support reasoning. It describes how to ensure that documentation is occupation-centred and client-friendly. Documentation is a requirement of being a professional. Writing progress notes is a way of tracking occupational therapy students' reasoning and actions for themselves and others. Progress notes can take many forms and it is important that students discuss the specific requirements of the setting with their educator. In some circumstances, it may be appropriate to generate group notes about a specific occupational context, such as a school playground, aged-care residence recreation room or hospital canteen. This might be particularly relevant if students are working to make universal changes to a setting for the benefit of all clients. The occupational performance analysis directs us to consider how the person performed overall during the observations of their occupational tasks. The potential causes of the performance challenges will guide us to consider suitable intervention approaches.
