Case study 5.1 Mr Hough is a 50-year-old who runs a small and ramshackle sheep farm. He attends in the autumn complaining that he has already seen ‘that young doctor’ and has had two lots of antibiotics without effect. He has come back about the X-ray that the GP registrar arranged and wants you to get him some strong antibiotics to ‘clear it up’, like he usually has each winter. The chest X-ray is clear.
