ABSTRACT

This chapter examines interdisciplinary approaches and best practices for models of care. Aging well requires a wide range of ongoing services and management of transitions as the needs and health status of older adults change. Many older adults need a blend of medical services and social supports, but there is no standard system for accessing these services. In this context, innovative models are being developed across the U.S. that offer promise for addressing the various challenges facing older adults and their caregivers, with the most notable of these expanding their services into additional communities.
