ABSTRACT

This chapter addresses issues that are transforming the environment and structure to population health management (PHM), suggests structural ideas, and ways to move healthcare leaders' organization into the new environment. The change from fee-for-service (FFS) to PHM is not just about a change in how people get paid, it requires significant changes in the culture of healthcare, how it is provided and how it is consumed. It requires full engagement of providers, payers, purchasers and patients. When providers take on the ownership of a population, which often means the population/communities they serve, the responsibility takes on a broader meaning than the delivery of healthcare. The chapter provides a view of a structure and operations of a Physician Hospital Organization (PHO). Peter Wood and the organization he worked with, the Maine Physician Hospital Organization (PHO), continued to thrive through innovation while other like organizations in other parts of the country failed.
