ABSTRACT

The actual capacity of a resource to complete a given task or set of tasks is dependent on a number of factors, all of which can constrain or expand the resource's capacity for workload. As the scope, scale, and clinical challenge(s) of the Care circle networks (CCN) have been decided and the potential resources mapped, this chapter focuses on to resource selection and/or detailed predictive analysis on population health. Physicians are perhaps the most critical component of a CCN, yet they are also the most constrained and the least likely to want to take on additional responsibilities, tasks, risk, and communications. Importantly, the CCN can be constructed in such a way as to support many physicians whose patients are primarily poly-chronics via a single communal resource pool. If multiple resources are required for a given patient, it is vital for the CCN's functionality and long-term viability that a team environment be created.
