ABSTRACT

A right hemicolectomy performed to remove the tumour and all of its draining lymph nodes. This chapter describes the preoperative preparations, procedure and complications occur at intra-operation. An intravenous prophylactic antibiotic is given to the patients immediately prior to surgery. The patient is placed in a supine position on the table, with a 20° tilt to the left. The peritoneum is divided 2 cm lateral to the ascending colon, from the caecum around the hepatic flexure. The bowel mesentery is transilluminated to identify the ileocolic artery, right colic artery and the right branch of the middle colic artery. The specimen is sent for histological examination with the right half of the greater omentum, which is also removed with the specimen. The complications occurs intra-operatively are damage to the right ureter in mobilising the colon, tension in the anastomosis due to inadequate mobilisation, and ligation of the superior mesenteric artery.
