If we write less, are we still ‘covering our backs’? Because standardised recording frameworks are provided, it is easier to confirm the care given rather than needing to think about devising individualised plans from scratch. It takes less time to write, so there’s more chance of records being complete. Writing concisely and with implicit guidelines is much better than not writing at all. In a court of law, if it is not documented, it hasn’t been done.
