ABSTRACT

This chapter considers two ways in which traditional hospital services can be shifted into high quality care in the home. Hospital care at home is the term used to describe schemes which bring into the home intensive levels of care associated with acute hospitals, supplying medical, nursing and rehabilitation services as well as social support and equipment. Broadly, there are two organizational models for hospitals at home. One is the creation of a specialized team, usually hospital-based, often linked to a surgical specialty, which may seek to involve primary and community health staff. The other model which is described here by Liz Haggard, is a community-based service for a defined geographical population. KITTZ is an organization with its roots in community-based health services. It works as an independent agency to create more and better opportunities for independent living and for care and treatment at home.
