Let's get this right. A mistake is an error, an incorrect idea, or something incorrectly done. It is not a capital offence, a heinous crime or an occasion for a public flogging.

In the best of well-run organisations, from time to time, things will go wrong. Provided it doesn't happen too often, it is not a sin. What is a sin is getting something wrong, not doing something about it, not fixing it, or worst of all, not knowing something has gone wrong.
