ABSTRACT

You are reviewing patients in the medical outpatient clinic. Your next patient has been referred by his general practitioner with the following letter. Please read the letter and then review the patient.

Dear Doctor,

Re: Mr Rupert Eldridge

Age: 82 years

Please advise on the management of this elderly gentleman. I have known him for many years and he has always been a sharp-witted man with an immaculate dress sense. I was asked to see him at home by one of his children because he had been unable to wake him up. Upon my arrival he had woken up and appeared orientated but clearly not his usual self. He was reluctant to be admitted to hospital but agreed to basic investigations and a referral to your clinic. Blood tests showed a normal full blood count, urea and electrolytes, serum glucose and liver function tests. A urine dipstick demonstrated mild proteinuria but urine cultures showed no growth.
