ABSTRACT
A 67-year-old man is referred by his GP to the neurology outpatient department with a history of several weeks of weakness in his limbs. His previous medical history includes type 2 diabetes mellitus for 15 years. He is an ex-smoker, having smoked 15 cigarettes per day from the age of 21 to 55.
