METHOD Compare reectivity between the two sides on a single image. Obtain transverse and longitudinal images.

APPEARANCE The testes are homogenous and of medium level reectivity. The mediastinum testis is a highly reective linear structure in the posteriorsuperior aspect of the testicle draining the seminiferous tubules of the testes into the rete testis. Drainage from here is via the epididymis to the seminal vesicles. The rete testis is a low-reective area at the hilum of the testis with ‚nger-like projections into the parenchyma. Apart from these projections, the parenchyma of the testis should remain of homogenous reectivity. The appendix testis is present in the majority of patients, most commonly at the superior testicular pole or in the groove between the testis and the head of the epididymis medially. There is marked variation in its size and appearance; it is usually oval, although a stalk-like structure is occasionally seen.
