ABSTRACT

A 41-year-old male has a 6-week history of fever and night sweats. He has lost 2 kg in weight. He has had a recent cough with productive sputum, but his fever has failed to subside after a course of antibiotics. He smokes 10 cigarettes a day and drinks 16 units of alcohol a week. On examination, he is pale and looks unwell. There is no palpable lymphadenopathy or splenomegaly. Investigations show
