ABSTRACT
A 76-year-old man has a 2- to 3-week history of headache and lethargy. He smokes 15 cigarettes daily but does not have a history of chronic lung disease. His appetite is good and his weight is steady. He has also noticed generalised pruritus after a hot shower.
