ABSTRACT

This chapter shows that how to assess risk in a reliable and consistent way, using evidenced-based and peer-reviewed clinical tools and frameworks to structure and record the assessment which will then help to inform the corresponding safety plans and treatment goals. It discusses the importance of asking and exploring for the presence of suicidal thoughts, and the importance of co-creating actions with the patient and carer to mitigate the risks identified. The chapter explains the relationships among the risk factors relating to self-harm and suicide. It explores the importance of the craft of caring and its central role in guiding practice. The chapter explains how national guidelines and policies help to inform the craft of caring for those at risk and those with suicidal thoughts. It discusses the strengths as well as the limitations of risk factor identification and assessment frameworks, tools and approaches.
