ABSTRACT

This chapter describes the appraiser understand and uses the appraisal paperwork effectively. It focuses on the facilitation of Form 4 and the personal development plan (PDP). The Form 4 is the summary of the appraisal discussion and is a vital component of appraisal. In most appraisal systems it comprises the written Form 4, with a separate tabulated PDP. All appraisers need a good understanding of how to facilitate a PDP that will be useful and effective for the appraisee. It has been suggested that all the learning objectives in the PDP should be presented in terms of SMARTIES. In practice, it is very rarely used in most appraisal systems. The guidance notes are the same for both primary and secondary care. They emphasise the optional nature of the framework for keeping a fuller account of the appraisal discussion and suggest that its use would be to inform the next appraisal round.
