Events such as unexpected deaths due to misdiagnosis or emer­ gency situations in the practice that have not been dealt with appropriately can put an enormous strain on the whole practice, and may lead to feelings of inadequacy and failure among members of the practice team. All o f the parties involved need support, and it is important to find out what went wrong in order to learn from any mistakes that may have been made. Significant event audit is very important for dealing with these stressful events together rather than in isolation, and you should try to encourage this in your practice if your colleagues find it difficult to address these issues.
