The NHS culture is not - by and large - one which encourages reporting and analysis.1

The Trust aims to achieve a culture in which clinical excellence and high quality is central to service delivery. We recognise that this will only be achieved in a learning culture, not one o f blame so that quality is integral to all aspects o f the Trust's work. This includes the willingness to em brace constructive criticism and new ideas as well as a determination to break down any barriers which may exist.2
