ABSTRACT
The following audit questionnaire will give the reader a chance to assess his/her practice’s performance against the standards set out in the Caldicott Report and the Data Protection Act. This questionnaire should assist in finding areas that are in need of improvement and help to formulate the practice strategy to improve on confidentiality and data protection.
